Many people have baggy and puffy lower eyelids. Lower Eyelid Surgery (Blepharoplasty) is the removal of excess fat with or without skin tightening, which can drastically minimize the appearance of puffy lower eyelids that make everyone look tired and worn down. Dr. Lees's Lower Eyelid Surgery Method:

To minimize recovery and complications from scarring, Dr. Lee always removes the excess fat that is causing the puffiness from the inside of the lower eyelids (transconjunctival approach). This method enables faster healing and if the skin is not tightened, there is no visible scar.  

If excess skin tightening is necessary, Dr. Lee's has a unique approach that requires minimal incision and thus recovery. Patients who do not need the skin tightening procedure can expect their post operative recovery to be only 7-10 days (being compliant with our post operative instructions) while patients with the skin tightening can expect to have noticeable improvement with the resolved puffiness within the same 7-10 days period and few months for the small incision to disappear- varies with each individual but the scar is so minimal that very few people ever mention it. Fat Removal + Skin Tightening 

Case #1: Female patient in her late 50’s was recommended to have the skin tightening procedure due to her very thin, sun damaged and crepey skin which would most likely not tighten on its own. We told her that her pigmentation would not be resolved with surgery and recommended that she have the TCA peel on her face and lower lids to help repair and exfoliate her sun damaged skin. 

Fat removal via internal incision or transconjunctival method is THE MOST REQUESTED PROCEDURE due to the permanent results achieved with minimal recovery time. An incision is made inside the lower lids and the fat is removed from that incision. If one has good skin elasticity, the lower lids skin will retract and tighten on its own with time. During your consultation, if Dr. Lee feels that the quality of your lower lids skin will not retract on its own and will leave unacceptable wrinkles, he may recommend tightening the eyelid skin surgically. Surgically tightening the eyelid skin will help to achieve a tighter and smoother appearance but a noticeable scar can be very visible for up to 6 months or even longer, and if scrutinized, may be visible to you forever.

Example of Fat Removal + Skin Tightening

Recovery: 

You will have double vision after the surgery for 24 hours due to the anesthetic injection. After 24 hours, your double vision will subside. The most difficult part of this recovery will be the compression tape that you MUST keep on for at least 7 days. You can shower and wash your face the day after your procedure and the tape will not come off. Please avoid any strenuous activities for at least 2 weeks and no swimming is allowed if you put your head in the water for at least 30 days. We will provide instructions for postoperative care after your surgery along with Dr. Lee’s personal cell number to contact him for any emergencies or concerns. 

Lower Blepharoplasty Cost:

All surgical prices are subject to change, and you will be quoted our current pricing when you schedule the surgery. Currently, the price for transconjunctival fat removal surgery is $6,000 (fat removal to deal with the puffiness or "bags" using an incision inside the eyelids). This procedure is our most popular method of treatment due to its minimal recovery, approximately 7 days of wearing a very visible compression tape without a noticeable scar. This method is also the best preventive measure to prevent the fat pads from stretching the skin even more as you age and the fat pushes outward even more. These prices do not include hospital fees- most of our patients have this procedure in our office surgical suite unless they have serious heart related issues or anxiety. If you elect to have the procedure in an outpatient surgery center, you will incur add'l facility fees.

Lower lids surgery does NOT resolve with the following issues: 

  • Your natural hollowness/sunken tear trough and cheek anatomy caused by natural aging process and your unique bone structure- once the fat is removed, you may notice your natural anatomy now that the puffiness is resolved; fillers to this area is Dr. Lee’s most popular treatment area and MOST of these patients have never had lower lids surgery. Hollowness to the lower lids/cheeks is a natural progression of aging and why it’s such a popular procedure. The cost for filler treatment ranges from $950 per syringe with Dr. Lee and $700-$800 with other providers based on which product is used and lasts approx. 1 year. If you desire to have filler treatment after lower lids surgery, you will need to wait approx. 2 months after to have the treatment performed. **approx. 10% of Dr. Lee’s lower lids surgery patients return for tear trough filler after surgery; most of his lower lids filler patients are NOT his lower lids surgery patients 
  • Skin pigmentation issues- if your skin has dark pigment, you will need to address the skin separately from surgery. A combination of HQ bleaching cream(sold at our office and is prescription grade at $135) and also laser treatments may help with this issue.   • Visible veins/bluish tone under skin- this will NOT improve with surgery since the veins are visible due to your thin and translucent skin 
  • Malar bags/fluid collection on cheeks- this condition is difficult to improve. The cheek skin below the eyelids can become edematous and swell. The skin in this area has become loose and unwanted fluid to distend the tissue. Dr. Lee has come up with a treatment that often helps with this condition but may need to be performed on an ongoing basis to maintain the result using radiofrequency to heat and firm the tissue. The cost of the treatment with our nurse is $250 per treatment or $750 for a pkg of 4 spread out every 2 weeks; recommended treatment is min. 4 but ideally 6-8 treatments every 2 weeks and maintenance as needed. 
  • Skin laxity/texture- skin tightening procedure will help to create a smoother appearance by pulling the skin but comes with the cost of the very visible scar and long recovery; skin issues such as laxity and stretchiness is something that is unique to your anatomy so can not be treated with surgery. You can however improve it by continuous medical grade treatments such as peels, lasers and ongoing use of medical grade products that stimulate collagen growth and exfoliation. Dr. Lee has come up with a treatment that helps improve the skin texture but may need to be performed on an ongoing basis to maintain the result using radiofrequency to heat the tissue.
