The previous surgeon also removed too much fat and caused a sunken look that is not aesthetically pleasing especially for a man. Fortunately, the patient had some excess skin so Dr. Lee was able to remove the scar created by the previous surgery, lower the crease height and also relocate fat around the eyes to the crease line to help resolve the sunken appearance. Dr. Lee advised her that she had a mild ptosis on her right upper lid (weakness of the eyelid muscle that does not allow the eyes to open as wide as her left). Also, her right eye itself is noticeably smaller then her left- overall anatomy of her right eye is smaller then the left. Once the crease was created, the asymmetry and ptosis was unveiled even more. She was happy with the result of her left eye but did not like her right eye result. She wanted the right eye to open more like the left and wanted it to be as big as her right. Dr. Lee advised her that it is her muscle causing her right eye not to open as much as her left and does not have anything to do with the crease- the height of her crease from lash line is very similar. He also advised her that the overall size of her eyes can not be corrected by just cosmetic procedures. A crease is simply a line created that follows the natural shape and curvature of ones eyelids. There are numerous factors that is causing her asymmetry- 1) lower right brow 2) shorter epicanthus (inner corner) 3) right brow bone sits lower which causes her eyes to be smaller. ***the brow impacts greatly on the height and shape of the crease created so noticeable brow asymmetry will cause the crease to appear smaller or larger depending on the brow height. The patient elected to have only the double eyelids and epicanthoplasty procedure only WITHOUT the brow procedure. This is the reason why her right crease is slightly lower then her left. (Post op photo was taken 1 month after surgery; her epicanthoplasty scar is still visible and took 3-4 months to improve)
